- ISSN number.
- Number (volume/number or month/year) subscription is to begin. Unless specified, subscription will begin with the next published issue after order is received.
- Full postal address of subscriber, including zip code. Note: For a change of address, give both NEW and OLD addresses.
Please allow six weeks for delivery of first issue. Subscriptions are processed after payment is received. Checks should be payable to Heldref Publications, or subscriptions can be charged on VISA, MasterCard, or American Express.
